Medical Records Request

 

Our facility has partnered with Vital Records to process all of our Medical Records requests via a secure online portal. Requestors can click on the link below to submit their request electronically online. Once a request has been submitted, it's status is easily tracked online in real-time. Once complete the records will be provided electronically.

To check the status of a submitted request, please call Vital Records at 800-359-8520 option 1.


If you are a patient and would prefer to submit a written request please complete the Patient Medical record release form and fax it to the office at (951) 698-5213.
